Selected Case Study
Advancing Cultural Humility Through Simulation-Based Learning in Nursing Education
Context
Conducted with pre-licensure nursing students across multiple levels of training, this work brought together early-stage and near-completion students within a shared high-fidelity simulation environment, emphasizing peer learning across experience levels. Developed in collaboration with a certified healthcare simulation educator (CHSE), the simulation lab experience incorporated structured debriefing using a three-phase model (reaction, analysis, summary) to support reflection on clinical decisions, communication, and relational engagement. The design integrated technical skill development with relational and cultural awareness, allowing students to engage in the complexities of patient care in diverse contexts.
Gap
Within a relatively homogeneous geographical area, students had limited exposure to culturally diverse patient populations during their clinical experiences. This created a need for more applied opportunities to engage cultural differences, strengthen communication, and develop cultural humility in practice. The goal was to create learning experiences that required students to make real-time clinical decisions within diverse patient scenarios, while maintaining high standards of care across cultural contexts.
Approach
The approach combined assessment, simulation, and structured reflection to support applied learning. Students completed a pre-assessment, including the Transcultural Self-Efficacy Tool (TSET), to establish a baseline understanding of cultural awareness and communication. Learning was introduced through a series of short video vignettes, each followed by facilitated debriefing using a three-phase model (reaction, analysis, summary). These sessions focused on identifying missed cultural cues, strengthening communication, and integrating patient context more effectively into care. Students then participated in a high-fidelity simulation, working in mixed-level groups with patient scenarios that included both clinical and cultural factors. The experience concluded with a structured debrief, allowing students to reflect, exchange perspectives, and learn across levels of training. A post-assessment was administered two weeks later to evaluate shifts in students’ confidence and ability to engage with culturally diverse patients.
